Breast augmentation is perhaps the best known and most talked about cosmetic surgery. It remains the most performed and the one that most affects the psychophysical well-being of patients even before the aesthetic aspect.

 

More than once I happened to hear patients who have had this procedure say that the money they spent for the surgery were the best they spent in their lives.

An important indication of how many implications the simple increase in breast size has.

 

Aside from all the information that will follow, I urge  all the patients interested in breast augmentation to book a proper in person medical examination, because only by personally evaluating the initial situation, the anatomical characteristics, personal preferences and any problems posed by the individual case, it’ll be possible to give a correct indication and establish the personalized planning for their particular case.

 

Breast augmentation was not invented to be performed according to the surgeon's taste, it’s a surgical plan based on the specific characteristics of each individual. In terms of technicques, through this procedure, essential volumes are restored for the overall harmony of the female body, which must respect the canons of a natural look, proportion and consistency suitable for each individual patient.

INDICATIONS AND SELECTION PARAMETERS

 

Small , empty (post pregnancy) or asymmetrical (congenital, post-surgical) breasts may benefit from Breast augmentation. Although it is not uncommon to intervene to increase the volume of normal-shaped and fairly large breasts. The procedure involves the insertion of silicone gel implants in a sub-glandular or partial sub-muscular plane (dual-plane) depending on the indications that are evaluated during the medical examination. The prostheses are inserted through small accesses at the level of the mammary sulcus or from the lower edge of the areola (hemi-periareolar access), the healing will then make these scars almost invisible. In both cases the gland  isn’t compromised and therefore breastfeeding is also not affected. 

 

In case it’s necessary to perform a small lift of the breast and areola, a complete peri-areolar incision (round-block) is made. Breast augmentation must be a "plus" for patients and having a more voluminous and proportionate breast must in no way be a limitation. The operation takes place under general anesthesia or sedation and lasts about 90 minutes.



THE IMPLANTS: all the details

 

The prostheses used are the fundamental element to be able to guarantee a safe intervention in the short, medium and long term. The choice of size depends on the patient's initial parameters and these indications must be followed in order not to risk inserting prostheses that are too small or too large. The latter will  in fact lead to less natural results, trans-cutaneous palpability and the breasts will be strongly subject to the force of gravity,thus accelerating sagging over time. 

 

The implants we choose are the latest generation silicone gel implants. The gel is cohesive and for this reason, even if the external envelope were to break, the gel would not come out and would not spread into the breast pocket, remaining confined within the implant itself. The external membrane of the prosthesis is textured (rough) with a double layer, in order to guarantee less mobility, less peri-prosthetic capsule formation and very high stress resistance. 

These characteristics then make the maneuver for the rare cases of removal and replacement much simpler, quicker, easier and safer, to the benefit of the patient. All the implants used are covered by a warranty on the material for their entire lifetime. This means that in the event of breakage due to a manufacturing defect, the manufacturer will cover the cost for the replacement of the damaged implan. 

Obviously, traumatic or surgical events that affect the integrity of the material are not covered. The durability of the implants can be considered unlimited in principle.

POST-OP

 

Functional recovery is relatively rapid; from the moment of discharge ,which takes place in the afternoon or in the evening after the surgery, the patient can walk and move freely and they only need to be careful when moving the arms. Specifically, they’ll be able to do all the ordinary movements (eating, drinking, washing hands and teeth, getting dressed) as long as they avoid sudden and quick actions. As a guideline, it is sufficient not to raise the arms above the shoulder line.

 

If drains have been placed during the surgery, they are removed either the evening before discharge, or at the latest the following morning during a quick dressing. After 5/7 days the adhesive plasters that make up the post-operative dressing are removed and the wounds are disinfected and re-medicated. At this point the patient is not wearing patches and has no other impediments than the simple post operative bra and the attached elastic band. The gain in comfort is substantial.

 

After the one week mark, wider movements are possible, always respecting the aforementioned common sense rules. Patients with a sedentary job can undoubtedly resume activity, those with a job that involves direct effort with the arms will do better to wait an extra week.

 

The next dressing is booked after another 7 days. All dressings and any external stitches are removed (normally the sutures are all retractable and there are no external stitches).

 

From this moment, in theory, the patient will be able to resume driving, bathing or showering and will not have to take medications of any kind (except for some specific clinical cases). The 15-day mark allows a gradual resumption of all social activities and even the most strenuous work activities. A final check-up is set at 15 days from the previous one (1 month after the operation) and coincides with the first real assessment of the patient's conduct at home and also of the evolution of healing.

 

After this term, most patients have already regained considerable confidence with the new breasts and say they feel at ease, in fact very often they also resume sports; first light and then more intense. 

Breast augmentation (regardless of the technique used) never affects the ability to play sports in general, but neither does it affect the strength or mobility of the arms, so much so that over the years there have been many professional athletes who very quickly re-enter the relative disciplines of origin.

 

In the continuation of the convalescence, the patients usually report that they can sleep comfortably on their stomach after about 40 days. I recommend check-ups after 3-6 and 12 months.
